I used to work for a large home shopping company. Part of my job was looking after the database and systems responsible for selecting people to be marketed to - including through our telemarketing system.
We took the TPS seriously. It only prevented us from calling non customers, but company policy was to not call customers if they were on the TPS list even though we were legally allowed to. We went to a lot of effort to make sure people registered on the TPS lists were removed from all calling lists promptly.
If people are on the TPS list it's because they don't want to be bothered by phone calls. Respect that. Not only do you risk a fine but you could annoy people who might otherwise have become customers. Big companies that accidentally call a couple of TPS numbers might be told to be more careful. Someone ignoring the TPS list might be made an example of.
